Is Vagus Nerve Stimulation Safe? Who Shouldn't Use It

The short answer: transcutaneous vagus nerve stimulation — the non-invasive kind delivered through the skin at the neck or ear — is generally well tolerated in healthy adults, and reported side effects in the published literature are usually mild and local: tingling, skin irritation, occasional lightheadedness. It is not suitable for everyone, and the exclusions matter more than the reassurance.

Who should not use one without medical advice

Side effects that are normal

  • Tingling, prickling or a fluttering sensation at the contact point during a session. This is what stimulation feels like. It should be noticeable but never painful.
  • Mild skin redness where the electrodes sat, fading within an hour.
  • Feeling sleepy afterwards. Common, and generally the point.
  • A yawn or two mid-session. Also common.

Reasons to stop and reassess

  • Pain, rather than tingling, at any intensity setting
  • Lightheadedness or dizziness that does not pass within a few minutes
  • Any change in heart rhythm you can feel
  • A persistent headache after sessions
  • Skin irritation that has not settled by the next day

None of these are common, and all of them are a reason to stop using the device and speak to a doctor rather than to push through at a lower setting.

Practical safety habits

  1. Start at the lowest intensity. Higher is not faster and not better. Turn it up only until the sensation is clearly present.
  2. Use the gel. Conductive gel is not an accessory — it is what spreads the current evenly across the contact area. Dry contact concentrates it, which is the main cause of skin irritation with these devices.
  3. Don't sleep in it. Sessions are minutes long. These are not designed for overnight wear.
  4. One device at a time. Don't stack a vagus nerve stimulator with a TENS unit or another electrical device in the same session.
  5. Clean the contacts. Wipe the electrodes and your skin after each use. Dried gel is the usual reason a device starts feeling patchy.

The regulatory picture, plainly
